When you place your order on-line PhlebotomyServices.com will refer you to the Patient Service Center (PSC) blood draw site clinical laboratory nearest you. There are over thousands PSC's nationwide, providing a location close to you. (We need your zip code to locate a PSC near you) We work with many testing laboratories in towns and cities in all 50 states. PSI provides the necessary order(s). You must have the orders and requisition from PSI prior to going to the lab/PSC, a representative can discuss this with you by phone.      

 YOU MUST HAVE THE ORDER/REQUISITION BEFORE YOUR BLOOD TESTS CAN BE DRAWN.

Your zip code will help us determine which lab is most convenient. You will be given information as to fasting requirements, address information, business hours, etc. We then send the necessary requisition/order to the PSC with instructions.

Your blood is drawn at the PSC and then processed by that lab. After your blood is drawn at the lab's PSC, the results are sent to you confidentially with a description of the test(s) you received. If instructed by you, we will also fax a copy to your Physician or healthcare provider with a signed HIPAA Release. Nothing will be sent to anyone but yourself unless we have a written HIPAA Release, signed by you.

You may pay for this service online by credit card.

All orders are refundable within 12 months of order date less a $25.00 cancellation fee.
